General Liability Release: I authorize my child to participate in all camp activities and amusement devices including but not limited to inflatables/water rides, hamster wheel, obstacle course, bounce houses and foam machine. I understand that these activities and play on an amusement device entails both known and unknown risks including accident and injury. I acknowledge that I am solely responsible and liable for any hospital, medical or other costs arising out of any bodily injury or property damage sustained through the student's participation in these voluntary camp recreational activitities. Photo Release: In addition Living Water Church/Camp 22.6 has my permission to use my child’s photograph publicly in VBS materials. I understand the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use. I acknowledge that I have read and understand the risks set forth herein and accept full responsibility for the registered student
